Where do I find the forms?
We will alert you by email when forms for the season are open and ready for your attention! Then, log in to your online Pemi CampInTouch account and go to “Forms and Documents” OR login to your mobile Campanion App (available on the Apple App Store and Google Play) and go to “Forms.” (Read more here on how to access your online account.)
When are forms due? May 31!
All staff forms are due by May 31. Thank you for honoring this deadline! Over 2,000 forms are submitted for campers and staff, and this deadline enables us to prepare a safe and happy season for the entire Pemi community.
How do I submit forms?
After you hear from us that forms are open and ready for your attention, log in to your online Pemi CampInTouch account and go to “Forms and Documents” OR login to your mobile Campanion App (available on the Apple App Store and Google Play) and go to “Forms.”
Some forms can be completed online. Others must be printed, filled out, and uploaded to your account (or faxed to the number below). Specific directions for each form are provided below.
Please do not mail or email forms. All “print” forms must be uploaded to your online Pemi CampInTouch or Campanion App account or faxed to the number on the form.
Directions for Uploading Forms
If uploading through CampInTouch: Scan the form as a PDF (no jpegs or tiffs) using an app on your phone (like CamScanner) or a printer/scanner. Login to your account, go to Forms & Documents, and click on the “upload” symbol to the right of the form. Upload the PDF.
If uploading through Campanion (available on the Apple App Store and Google Play): To access mobile forms, navigate to the left side menu in the app and select “Forms.” Once there, you’ll see each form and its due date. You can complete, save, and scan/upload forms directly from the app.
Directions for Faxing Forms
Fax toll-free to the number on the form: 1-866-428-1917. Faxed forms are routed automatically to your online account via the unique barcode that was generated on the form when you downloaded or printed it from your account.
There is no need for a cover sheet due to the barcode on the form, which is unique to you. The system reads the barcode and routes the form automatically and securely to your account.
Faxed forms are digitized and we receive an alert that it has been directed to your account.
Direct Deposit Form (optional) - Print
- Optional.
- If you would like to be paid by direct deposit (or TransferWise for international staff), please print, complete, and upload this form to your account. Direct Deposit forms need to be received at least 6 days before the first pay date (there is processing time on the bank’s end between the submission of form and the initial direct deposit).
Health History Form - Online
This form is required for all staff. If you are a returning staff member, be sure to review your previously entered information to confirm its accuracy.
- Log in to your account and complete this form online.
- Have your insurance information/card(s) handy for reference.
- You can save your work and return to the form, but only fully completed pages are saved.
- A full immunization history/record for staff is not required. Instead, you can indicate that immunizations are up-to-date. However, you will need to provide: 1) date of your last Tetanus shot (referred to as DTAP/td on immunization records), 2) Tuberculosis (referred to as TB/PPD) test date and result (positive, negative) or “not tested,” and 3) any COVID vaccination/booster information requested.
- The information you enter on this form carries over from year to year, for convenience in submitting the form in subsequent summers. However, please verify that information provided for previous summers—especially on medical conditions—remains current. If you checked a box last year to indicate a condition, allergy, or medication, a separate box opened for you to provide additional information. This year, please un-check those that no longer apply, or confirm each of these conditions by clicking the box again.
- You can return to this form and update it at any time after it is submitted.
- Your signature at the end indicates that the information is accurate and current.
I-9 Employment Eligibility Verification – bring appropriate ID to camp!
- All new staff and returning international staff will complete an I-9 form, but we will do this together after you arrive at camp. There is no need to fill out an I-9 form before arrival at camp. But! Read on…
- However, you must bring acceptable ID to camp. For example, either a passport, OR two forms of ID (such as a driver’s license AND social security card). For more information about acceptable ID options, please see page 3 of the form.
- Form I-9 is used for verifying the identity and employment authorization of individuals hired for employment in the United States. All U.S. employers must ensure proper completion of Form I-9 for each individual they hire for employment in the United States. This includes citizens and non-citizens.
Parental Permission Form (for under 18 only) - Print
- For staff under age 18 only!
- Have your parents complete this form, then upload it as a PDF to your account.
Staff Authorization Form - Print
- All staff must submit a new Staff Authorization form every year, to ensure that we have current information in case of emergency. This form provides “permission to treat” in case of emergency.
- Log in to your account and print this form. Follow the directions on the form.
- This form must be ONE SINGLE-SIDED page uploaded as a PDF to your account. The images of your insurance cards and your signature must be all together on one page. The back side of the page should be blank.
- DO NOT upload 2 separate pages!
- A parent or guardian must sign for staff under age 18.
Staff Bio & Photo - Online
- Required for all staff.
- Log in to your account and complete this form online.
- Our first summer newsletter to parents and the Pemi community includes the staff bio blurbs of our entire staff. Help us to introduce you!
- Three to five lines, approx 50–100 words. Consider telling us where you’re from, what your interests are, what you do or where you go to school, what you hope to do/teach at Pemi, or how many years you’ve been at Pemi.
- Click here to see last year’s bios!
- Please provide a current, close-up photo for uses such as cabin and staff group photos for Opening Day. A crop tool is provided on the form.
W-4 Employee’s Withholding Certificate - Print
- Required for all staff.
- Login to your account and print this form. Follow the directions on the form.
- You may upload just the completed Page 1 to your online account (or fax to1-866-428-1917).
- As an employer, Pemi is required to obtain a Form W-4 from you for payroll purposes, and will use it to provide a Form W-2 later on, so you can prepare and file your tax return.
- The information you provide on the W-4 tells us how much money you wish to have withheld from your paycheck for federal income taxes, and will affect how much tax you owe or are refunded when you file your annual tax return.
- INTERNATIONAL STAFF: if you do not yet have a U.S. Social Security Number (SSN), leave the SSN field blank, but go ahead and fill out the rest of the form and upload it.
