All About Forms

The easiest way to complete Pemi forms is through the mobile Campanion App. Campanion allows you to complete pre-camp forms and upload paperwork directly to your Pemi CampInTouch account. Download the Campanion app (available on the Apple App Store and Google Play) and log in using your Pemi CampInTouch email and password OR Log in to your Pemi CampInTouch account and go to “Forms and Documents.”

We use the information you provide with the greatest discretion, and encourage you to make us a full partner in planning for your child’s summer. Our office, counseling, health, and kitchen staff use the information you provide during the pre-season staff training and throughout the summer to help your child adjust to camp and to provide a safe and supportive experience. Doing forms can take a little time, but it’s not complicated, and we provide detailed guidance and tips. Always feel free to contact us with any questions you might have.

Remember: forms should not be sent via mail or email. Please upload forms directly via your online CampInTouch account, the Campanion App, or fax them to the number on the form. These are the most accurate and secure ways to deliver these forms directly to your son’s digital file.

All forms in the first round are due by May 1st. Thank you so much for honoring this deadline — we need this time to compile and prepare the over 2,000 forms we receive, towards ensuring a healthy, safe, and happy camp experience for your son!

Camper Recommendation Form (for first-time campers only)

For new campers, a camper recommendation from a teacher is required: please provide the link for this Camper Recommendation Form (GoogleForm) to your son’s teacher. The prompts address a boy’s readiness to live in a residential community and requests information from your son’s teacher that will assist us with cabin and counselor placement.

What forms do I need to submit?

After you hear from us that forms are open and ready for your attention, log in to your Pemi CampInTouch account and go to “Forms and Documents” OR login to your mobile Campanion App and go to “Forms.” You’ll find detailed information and directions for each of the following forms further down this page. But generally:

  • You will need to log into your online Pemi CampInTouch account OR mobile Campanion App and PRINT (then complete and upload/scan PDFs for) the following forms:
    • Pemi Code of Conduct
    • Parent Authorization Form
    • Physician Exam Form
    • Pemi Medication Authorization Form (Required for campers taking DAILY or ROUTINE medication/supplements at Pemi)

Directions for Uploading Forms

If uploading through CampInTouch: Scan the form as a PDF (no jpegs or tiffs) using an app on your phone (like CamScanner) or a printer/scanner. Login to your account, go to Forms & Documents, and click on the “upload” symbol to the right of the form. Upload the PDF.

If uploading through Campanion (available on the Apple App Store and Google Play): To access mobile forms, navigate to the left side menu in the app and select “Forms.” Once there, you’ll see each form and its due date. You can complete, save, and scan/upload forms directly from the app.

Directions for Faxing Forms

Fax toll-free to the number on the form: 1-866-428-1917. Faxed forms are routed automatically to your online account via the unique barcode that was generated on the form when you downloaded or printed it from your account.

There is no need for a cover sheet due to the barcode on the form, which is unique to your son. The system reads the barcode and routes the form automatically and securely to your son’s account. For siblings, be sure to download EACH individual boy’s forms; don’t make copies of one or scratch out the barcode. If the same form is used for both campers, the second form to be submitted will overwrite the first.

Faxed forms are digitized and we receive an alert that it has been directed to your camper’s account.

Should I email the forms to you? No - upload or fax.

How do I know if my forms have been received?

Log in to your Pemi CampInTouch account and go to “Forms and Documents.” A properly completed and processed form will be marked as “received,” and the form’s name will change from black to green (overdue forms’ names turn red!). If there is an issue with your submitted form, we will be in touch with you. NOTE: We review each form for accuracy and have close to 2,000 forms coming in for campers and staff, so it might be several days before they’re marked “received”!

In the mobile Campanion App, navigate to the left side menu in the app and select “Forms.” Once there, you’ll see each form and its due date. A properly completed and processed form will be marked as “received,” and automatically be moved under a new heading “Completed Forms.”

Forms for siblings have unique barcodes, so download seperate forms from each camper's account.

Each boy’s forms include a barcode on the lower right that is unique to him. In order to properly identify siblings, be sure to download EACH boy’s forms; don’t make copies of one or scratch out the barcode. If the same form is used for both campers, the second form to be submitted will overwrite the first.

Camp Aide/Waiter Application (optional)

  • Log in to your account and complete this optional form online through your Pemi CampInTouch account or the Campanion App.
  • For campers ages 15, 14, and older 13’s who would like a camp aide job, such as: Art World, Bugler, Health Center, Hill Top, Library, Lodge/Laundry, Mail/Flowers, Music, Office, Recycling, Archery, Balls/Fields, Basketball, Nature, Sailing, Shop, Tennis, Waterfront, or Water Skiing.
  • We strongly encourage our oldest campers to serve as waiters and aides! These are paying positions and serve as excellent leadership opportunities.

Camper Questionnaire

  • Log in to your Pemi CampInTouch account or the Campanion App and open this form for your son to complete.
  • Thank you for having your son tackle this form on his own, as a first step towards independent camp living! He and his counselor will go over this form together when he arrives at camp

Health History Form

General immunization policy: the NH Department of Health and Human Services (NHDHSS) requires ALL campers to be fully vaccinated and provide written proof of vaccination. Pemi expects all campers who are medically eligible to be up to date on vaccinations. Should your son(s) not be up to date on vaccinations, contact Director Kenny Moore (ken @ camppemi.com). When you (or your doctor’s office) submit your child’s Physician Exam form, you MUST include your child’s most recent immunization record with the form (all pages together as 1 PDF file/fax).

  • Log in to your Pemi CampInTouch account or mobile Campanion App and complete this form online.
  • Have your insurance information/card(s) handy for reference.
  • This form has several sections. You can save your work and return to the form, but only fully-completed pages are saved.
  • For parents of returning campers: the information you enter on the Health History form carries over from year to year. However, please verify that information provided for previous summers—especially on medical conditions—remains current. If you checked a box last year to indicate a condition, allergy, or medication, a separate box opened for you to provide additional information. This year, please un-check those that no longer apply, or confirm each of these conditions by clicking the box again.
  • This form can be updated after it is submitted, and should be if any information changes.
  • Your e-signature at the end indicates that the information is accurate and current.

IMPORTANT: A note about completing the “Medications” section of the Health History Form — You must enter all medications or other supplements that your child will take DAILY or REGULARLY at camp in the medications section of the Health History form. This includes liquid, inhaler, nasal spray, a medication patch, or rescue medication such as an epi-pen. We stock over-the-counter medications that provide relief/comfort for the occasional need, and you do not need to enter these.

  • In the medication section of the Health History form, check the box indicating your child will take a medication at camp. A pop-up window will appear.
  • Please accurately input the name of the medication.
  • “Dosage” – enter the dosage in milligrams (do not indicate a volume such as 5 mls or 1 teaspoon).
  • “Initial Count” – you may leave this blank. Our Health Center staff will complete this field if necessary.
  • “Start Date” and “End Date” – enter the exact start and end date of the course of medication. This will usually be the start and end dates of your child’s Pemi session, but could vary if your child is finishing a course of medication that he started before coming to camp (e.g. an antibiotic for an infection).
  • Indicate the mealtime that the medication should be administered. Only if it is medically necessary, and a specific time written on the prescription, will meds be dispensed at a different time of day. If a medication must be given outside of meal times, i.e. “other,” indicate the exact time and reason why in your instructions on the Health History form.
  • If medication is “as needed” (sometimes indicated by “PRN”), check the corresponding box and provide instructions as to when the medication should be used.

Parent Authorization Form

  • Log in to your Pemi CampInTouch account or mobile Campanion App and print this form. Follow the directions on the form.
  • This form grants your “permission to treat” in case of emergency. It will accompany your son when he is away from camp. Having the insurance information and “permission to treat” all on one page is critical.
  • Complete this form as one SINGLE-SIDED page only and upload to your CampInTouch/Campanion account or fax to the number on the form. If you upload multiple pages as separate PDF files, subsequent uploads will erase prior uploads.
  • All information must be on one side. The back side should be blank.
  • DO NOT upload multiple pages with insurance card images on separate pages.
  • A parent or guardian signature is required at the bottom.

Parent Questionnaire

  • Log in to your Pemi CampInTouch account or mobile Campanion App and complete this form online.
  • Your inputs as a parent provides insight for our staff during staff training as well as for cabin and counselor placement.
  • Please be forthcoming with any information that will help us help your son make a smooth and happy transition to camp, such as information on a learning difficulty, ADHD, bed-wetting, anxiety or depression, or a recent loss or major change in the family or your son’s life.
  • TIP: CampMinder will close the session for this form if it has been inactive for 15 minutes. It is best to complete this form in one sitting—or, if you’d like to give it more thought, draft your responses elsewhere and copy and paste them into the form.

Pemi Code of Conduct

  • Log in to your Pemi CampInTouch account or mobile Campanion App and print this form.
  • Follow the directions on the form.
  • A Pemi code of conduct to be read, enjoyed, discussed, and signed by you and your son together.
  • Upload to your Pemi CampInTouch account or mobile Campanion App or fax to the number on the form.

Permissions & Policies Form (Acknowledgement of Risk)

  • Log in to your Pemi CampInTouch account or mobile Campanion App and complete this form online.

Physician Exam Form

If your son has an exam scheduled after the May 1st deadline, please request that your physician complete and submit this form by the May 1st based on a prior exam. You may submit an updated form after the deadline, with immunization updates as appropriate or if any significant health changes come to light.
  • Physician’s own office-generated form is fine.
  • The physician’s office may fax this form and your child’s most recent immunization record to the number on the form.
  • For UPLOADS: all pages should be uploaded together as a single PDF file. If you upload multiple pages as separate PDF files, subsequent uploads will erase prior uploads.
  • Unique barcode on the form will route faxes directly to your son’s account.
  • Or, you may log in and upload the completed form and immunization record.
  • All pages need to be in a single PDF for uploads. Uploading individual pages will erase each earlier upload.

Pemi Medication Authorization Form (Required for campers taking medication at Pemi)

  • Log in to your Pemi CampInTouch account or mobile Campanion App and print this form.
  • Please take this form to your child’s healthcare provider for completion.
  • Use a separate form for each prescription or over-the-counter medication (OTC), including vitamins/supplements that are to be administered DAILY/ROUTINELY.
  • Medications, both prescription and non-prescription (“over-the-counter”) will be administered only with authorization by a licensed healthcare provider (physician, nurse practitioner, or dentist) and written consent from the parent/guardian. The parent/guardian is responsible for obtaining the authorization of the healthcare provider as well as providing the specific medication to be administered at Camp Pemigewassett.
  • The physician’s office may fax this form to the number on the form OR, you may log in and upload/scan the completed form to your Pemi CampInTouch account or through your mobile Campanion App.

Please Note: OTC medications needed for occasional, “as needed” administration are stocked and dispensed at camp (E.g. Tylenol, ibuprofen, cough medicine, decongestant, antihistamines, cortisone cream) and do not require this form. Please do not bring occasional “as needed” medications to camp.

Travel Form

This form is required for all campers, even if travel is by car. It is due 4 weeks before the start of your son’s session.

  • Please go to the Travel page first, for current information and to plan accordingly for arrival/departure by car or plane.
  • Log in to your online Pemi CampInTouch account or mobile Campanion App and complete this form online.
  • If you are flying with your son and are renting a car in order to deliver your son to camp, select the car icon (we only need flight info if a Pemi chaperone will be dropping your son off at the airport for departure).
  • Both arrival and departure must be completed for the form to be submitted successfully.
  • Use the dropdown menu to indicate if baggage will be with the camper, or shipped via UPS (otherwise the form will be considered “incomplete.”)
  • If travel plans change after you’ve submitted the form, please update the form. To update, click the “edit” button next to the bus, car, or plane icon that you’d previously selected.
  • If your travel plans change within 5 days of opening/closing days, please update the form AND contact us directly by phone or email.
